Shot records on Form 3231 can be provided by your Pediatrician.  Shot records are required at the beginning of the school year.  Shot records can be faxed to the preschool office at 678-208-1329 or emailed to joanie@cbccumming.org

If you choose not to have immunizations, we must have the following notarized exemption form 2208 on file prior to the first day of preschool.
